Event Wagon Application

This application form must be filled out completely and submitted with payment of the reservation FEE plus tax to the CH Recreation Department.  The Event Wagon is for use within the City of Columbia Heights and may not be moved once delivered. Refer to the Event Wagon Procedures link at the bottom of the form for further information.
 
Delivery of the event wagon will normally occur in the morning and picked up the following day. If the event is to be held on a weekend, the wagon will be delivered on Friday and picked up on Monday morning.
 
*The applicant acknowledges and agrees and specifically states that he/she has read and understands all policies, rules, and regulations relating to the Neighborhood Event Wagon and does hereby agree to defend and hold harmless the City of Columbia Heights from liability in case of accident or injury.
